What is Hallucination?

When a model generates information that is factually incorrect, nonsensical, or fabricated while presenting it as if it were true. Hallucination is a fundamental limitation of language models — they generate plausible text, not verified facts. RAG can reduce hallucination by grounding outputs in retrieved documents.
